As a Support Broker, you will be a Guiding Light for individuals seeking to enhance their lives through tailored support services. Your responsibilities will include:
Forging meaningful relationships with individuals with disabilities, understanding their unique aspirations and needs.
Collaborating with clients, their families, and a network of service providers to develop comprehensive support plans.
Assisting clients in navigating the array of services, programs, and resources available to them.
Advocating for employment opportunities that align with the strengths and talents of individuals with disabilities.

As a Companion, you will be providing direct support for a young man with a desire
to be a part of his community. You will have the opportunity to support him as he navigates self-direction and participates in meaningful activities such as volunteering, shopping, short hikes, and nature walks as well as community involvement. He needs assistance with making choices that support an independent and desired quality of life.
As a Direct Support Professional, you will be providing support to a 38-year-old with the desire to increase his independence and go on community outings. You will have the opportunity to encourage and assist him to take part in activities as needed. You will also have the opportunity to assist him with choosing different activities, ones he likes and even new ones to expand his interests.
As a Job coach/Enhanced Supports Professional, you will be providing direct support to a 32 year old individual with the desire to gain independence and enhance his skills at his place of work. You will have the opportunity to assist him with his job and build up his skills that come with his position. You will also have the opportunity to go out into the community with him which may include activities such as bowling, swimming (be willing to swim with him), social clubs, gym (be willing to work out with him), and thrift stores.
As an Enhanced Direct Support Professional, you will be providing direct support for a 41-year-old man with a desire to be a part of his community. You will have the opportunity to assist him as he navigates self direction and participates in meaningful activities such as baking, shopping, visiting fire stations. He enjoys having the opportunity to be integrated in the community and needs assistance with making choices that support an independent and desired quality of life.
We are looking for a reliable LPN (Licensed Practical Nurse) or RN
(Registered Nurse) to care for a young woman in her home and the community. LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It can be a demanding job but also very rewarding.
As an Enhanced Direct Support Professional, you will be providing direct support to a 28 years old individual with the goal of increasing their independence and developing relationships. You will have the opportunity to assist him with occasional grocery shopping, learning new meals to cook and cleaning. You will also assist him with going on some community outings such as activities or lunch on available weekends. Located in Harleysville, PA

As a Direct Support Professional, you will have the opportunity to assist a young man with mild intellectual disabilities to be involved in his community and local activities. This is a position for someone with a positive attitude who is patient, kind, and enjoys being a friend!
