Housing Transition & Tenancy Sustaining Services

Housing Transition and Tenancy Sustaining (HTTS) Services help people get ready for housing and keep their homes. Our services provide support to make sure individuals can be successful tenants, whether they own, rent, or lease their homes. These services are available through the Consolidated, Person/Family Directed Support (P/FDS), and Community Living waivers.

We will show various housing options to individuals with intellectual and developmental disabilities and their families, helping them understand the pros and cons of each. Our goal is to assist people of all abilities in choosing the features and options that best suit their needs.

The Fourteen Major Components of The Service Are:

1. Conducting a Tenant Screening and Housing Assessment

2. Developing an Individualized Housing Support Plan

3. Assisting with/or Identifying Housing Resources

4. Assisting with Maintaining Housing

5. Identifying Resources to Cover Expenses

6. Assisting and Arranging for and Supporting Details of the Move

7. Developing a Housing Support Crisis Plan

8. Assisting with Resources and Relationship Development for Community Integration

9. Assisting with all aspects of planning for housing

10. Working with the ISP team to identify any needed assistive technologies or home accessibility adaptations

11. Assisting with coordinating the move from congregate care or a family home to new home

12. Working together with other providers and unpaid supports

13. Assistance with identifying resources for household furnishings and utility assistance

14. Assistance in maintaining residency in their new home
